CAMFT Members:

1. Print the Application form:

• CAMFT Member Application 
• Mail or Fax the completed applicaton to CAMFT

or

2. Go Paperless! Login to the CAMFT Members Only section with your user id and password

• Once you are logged in , click on "Update my Profile"
• Once on the Member Profile page, click on   
• Read the statement and follow the instructions under How to Update Your CounselingCalifornia.com Listing to complete the process.

Non-Members:

1. CAMFT Non-Member Application 
(The application forms are in PDF format. If you do not currently have Adobe Acrobat Reader you can Click here for your Free download software.)

2. Fill in application

3. Mail the application to:

4. Fax the application to CAMFT at (858) 292-2666
